May perform one or more of the following duties:

Functional Description:
· Conduct home visits to inform recipients of services, application and eligibility process, initiate and complete needs assessment documentation for validation process, using Minimum Data Set-Home Care (MDS-HC) assessment or other assessment tools designed by the Department
· Obtain and review medical information and evaluations performed by various agencies and groups
· Gather existing evaluative and eligibility materials from various sources to verify the level of care needed and severity of medical condition; verify consumer’s disability/diagnosis
· Prepare package of required eligibility materials, including obtaining necessary signatures, and forward to the Bureau of Community Supports and Services (BCSS) or designee
· Participate in Continuous Quality Improvement (CQI) process

Education and Typical Years Experience
· Bachelor’s degree in health or human services, social work or related discipline
· Master’s degree may be substituted for two years of experience
· Minimum of three years of health related social and/or human service experience; at least one year experience working with mentally challenged or physically disabled persons
· Knowledge of healthcare preferred; Medicaid or Medicare experience also preferred


Special Requirements
· Effective verbal and written communication with internal/external personnel
· Ability to think critically and to be trained in other areas when necessary
· Ability to interpret data and make judgment calls
· Independence on work assignments requiring exposure to detailed and sensitive information
· Use of personal computer and phone for up to five or more hours daily
· Valid drivers license
· Frequent and/or prolonged travel
· Occasional reaching, bending, stooping, kneeling, or crouching
· Working knowledge of and experience in:
¨ Bureau of Community Services and Supports (BCSS)
¨ Bureau of Health Services Financing (BHSF)
¨ Department of Health and Hospitals (DHH)
¨ Medicaid, Office of Citizens with Developmental Disabilities (OCDD)
¨ Personal care services
· Understanding of healthcare, especially in areas related to mental retardation and developmental disabilities
